2027 IRMAA Brackets: Projected Income Limits & Part B Premium
Last updated · Sources: CMS, SSA, BLS
Your 2027 Medicare Part B and Part D premiums depend on your 2025 modified adjusted gross income (MAGI) — see the two-year lookback and what counts as MAGI. The first IRMAA tier is projected to start above $112,000 (single) / $224,000 (married filing jointly), up from $109,000 / $218,000 in 2026. Check your own tier with the IRMAA calculator.
2027 IRMAA income limits (projected)
Thresholds use the lower projection; the value in parentheses applies if SSA/CMS treat the missing October 2025 CPI-U differently. Surcharge columns are 2026 placeholders.

* 2027 dollar amounts are not yet published by CMS. The surcharge columns repeat the 2026 official amounts ($202.90 standard Part B premium) as placeholders; expect them to rise when CMS announces 2027 premiums. Thresholds are our projection from the statutory CPI-U formula; tiers 2–4 may land $1,000 higher (single) or $2,000 higher (joint), shown in parentheses. How we projected these.

2027 Part B premium: Trustees-based estimate
The 2026 Medicare Trustees Report projects a 2027 standard Part B premium of about $209.50/month (+$6.60 from $202.90). By law IRMAA enrollees pay 35%, 50%, 65%, 80% or 85% of program cost instead of 25%, so totals run about 1.4×, 2.0×, 2.6×, 3.2× and 3.4× the standard premium. Applied to 2026 ($202.90) this method gives $284.10 / $405.80 / $527.50 / $649.30 / $689.90 versus the official $284.10 / $405.80 / $527.50 / $649.20 / $689.90 (tier 4 within $0.10).

Part D IRMAA for 2027
Not yet announced. Part D uses the same income tiers; the 2026 surcharges were $14.50, $37.50, $60.40, $83.30, $91.00 per month (tiers 1–5), paid on top of your plan premium. Expect modest increases. More: Part B vs Part D IRMAA.
2027 COLA and your net Social Security check
SSA announced a 3.6% COLA for 2027 (October 14, 2026). If your Part B premium is deducted from your benefit, the net raise is the COLA minus the premium increase — about $6.60/month at the standard level on the Trustees estimate, more if you are in an IRMAA tier.
What changes when CMS announces
- Official 2027 standard Part B premium and deductible replace the $209.50 estimate.
- Official Part B and Part D IRMAA amounts replace the 2026 placeholders.
- Official thresholds settle whether tiers 2–4 use the lower or higher projection.
- If your 2025 income was unusually high because of a life-changing event (retirement, death of a spouse, divorce), you can ask SSA to use a lower year with Form SSA-44.

Questions
What are the 2027 IRMAA brackets?
CMS has not announced them yet. Projected from the statutory CPI-U formula, 2027 IRMAA starts at 2025 MAGI above $112,000 (single) or $224,000 (married filing jointly). Tiers 2–4 are projected at $141,000/$176,000/$211,000 single, possibly $1,000 higher; the top tier stays at $500,000 / $750,000.
What will the 2027 Medicare Part B premium be?
Not announced yet. The 2026 Medicare Trustees Report projects a 2027 standard Part B premium of about $209.50 per month, up $6.60 from $202.90 in 2026. CMS sets the final figure, usually in November.
What income year is used for 2027 IRMAA?
Your 2025 MAGI (adjusted gross income plus tax-exempt interest) from your 2025 federal tax return — a two-year lookback. If that return isn't available, SSA may use 2024 and correct it later.
Why might the 2027 thresholds differ from the projection?
The BLS never published an October 2025 CPI-U because of the 2025 government shutdown, and that month is inside the averaging window. Depending on how SSA/CMS handle it, tiers 2–4 could be $1,000 higher for single filers ($2,000 joint). The first tier ($112,000 / $224,000) is the same either way.
How much is the 2027 Social Security COLA?
SSA announced a 3.6% cost-of-living adjustment for 2027 on October 14, 2026. Part B premiums are usually deducted from Social Security benefits, so a higher premium or IRMAA tier reduces the net increase.
When will CMS announce the 2027 IRMAA amounts?
Typically in the fall; the 2026 figures came out November 14, 2025. We will replace these projections with official figures as soon as they are published.
